I'll do you the courtesy of giving an answer - it would be nice to have it reciprocated now and then.
By way of a simple personal anecdote as an answer, - during my apprenticeship days, I spent a few months in a power press shop, - even back in those days, every machine had an automatic mechanical guard that protected us operators as the press did its work, - it had the side effect of fractionally slowing down the cycle, and as the regular operators were piece-workers, time was money, so a few morons would disable the guard mechanisms.
Those of us who realised the stupidity of doing that wouldn't have dreamt of doing such a thing, even though we probably didn't need guards at all for the very same reason but they were an aid to our own commonsense.
A few (thankfully not life-threatening) accidents (to guess who?) in and the company decided to move to failsafe photo cell guards (even before the EU started to take credit for such things) - we were then all of us forced to sit and wait until the machine told us it was 'safe to proceed', - slowing the job down for everyone, and undoubtedly making the company less competitive.
